Community agencies may provide an interim social support service for seniors and persons with disabilities before, during and upon discharge from the hospital. This service is for seniors and persons with disabilities who have little or no personal or family support at home, and who do not qualify for the Home and Community Care Program. Talk to the hospital discharge planner about community resources that provide this return to home service.
